 The Daily Living Skills Aftercare Program supports graduates by offering ongoing structured support, community accountability, and early intervention tools that prevent relapse into crisis-driven behavior.

 

🧩 Target Audience

  • Graduates of the Huntors Helpers 24-Month Daily Living Skills Program

  • Waiver-eligible individuals who have met most or all ADL/IADL goals

  • Individuals who benefit from consistent follow-up, structure, and positive peer support

 

🕒 Program Frequency & Structure

  • Meetings: 2x per month (biweekly)

  • Session Length: 90 minutes

  • Format:

    • 1st Session: Skills Reinforcement & Peer Strategy Exchange

    • 2nd Session: Community Check-In + Real-World Scenario Labs

 

🔄 Core Goals of the Aftercare Program

  1. Maintain Consistency in daily living routines

  2. Reduce Regression by proactively identifying triggers

  3. Prevent Crisis Dependency through regular check-ins

  4. Build Peer Accountability and mutual encouragement

  6. Connect to Resources in real-time before needs escalate

A fun, hybrid program—covered by the Ohio Medicaid Waiver—that turns everyday moments into stepping-stones toward independence.

Introduction

 Every young person deserves the chance to live a life filled with purpose, independence, and meaning. However, for youth with developmental disabilities, transitioning into adulthood can be particularly challenging due to limited structure, inadequate support services, and a lack of trusted guidance after high school.

 

The Huntors Helpers Independent Provider-Led Daily Living Skills Support Program goes beyond being just a life skills course — it’s a transformative and healing journey. Created and led by Terrelle, a dedicated special needs mother, case manager, caregiver, and direct support provider, the program is deeply rooted in personal experience. It reflects a profound understanding of the challenges and triumphs faced by youth with developmental disabilities and their families.

 

Through structured biweekly group sessions and personalized weekly coaching, participants gain more than practical skills for independent living. They discover how to grow, how to lead, and how to heal — empowering them to embrace their full potential.
